Waiver Form (v 2026)
IBOLD has partnered with the City of Allen to host our league events. The City requires each participant in hockey league events to complete and sign a Waiver for every calendar year. We kindly ask each participant to help us satisfy the City's waiver requirements.
In order to participate in IBOLD hockey events, please complete and sign the attached Waiver and deliver it to an IBOLD League Coordinator.
